Abstract

Polymyxin B is first-line therapy for carbapenem-resistant organism (CRO) nosocomial pneumonia, but clinical data on its pharmacokinetic/pharmacodynamic (PK/PD) relationship for this indication were limited. This real-world prospective study in 105 patients used 295 plasma concentrations to build a population pharmacokinetic model with Phoenix NLME, then used logistic regression and Monte Carlo simulation to identify the PK/PD index most predictive of treatment efficacy. AUC/MIC was identified as the most predictive PK/PD index, with an optimal cutoff of 66.9 in combination therapy, and simulations indicated daily doses of 75-100 mg every 12 hours would achieve target attainment at clinically relevant MIC values. The findings support specific polymyxin B dosing recommendations for CRO pneumonia, with adjunctive inhaled therapy suggested for patients unable to reach target concentrations intravenously.
